[WANTED] 64 Ownercard high-res scan.

Hey guys, looking for a high resolution scan/photo of both sides of the owner card that would have come with a 64 new. There's a pocket in my manual that feels empty without it.

I found this pic after a Google search:

which I think is the card in question. Looking for a high res so I can reproduce it. I have a professional double sided color card printer so it will be easy to make with a good template. Thanks guys.

Will this work? The card is 2 1/8" x 3 5/16".

When you post, can you post a blank or generic version for others to modify for their own use? That would be great!

Can do! I'm going to make a card true to the original owner since I have all the details from the warranty card. And I'm going to make one with my info and purchase date.

I never found the exact font, but found one pretty close that I used.
